All plans include a 15% wellness incentive for all other dental treatment. This program is designed to help empower you to take control of your oral health.
*All benefits must be used within 12 months of the plan activation or be forfeited. Periodic exams/cleanings will be scheduled 6 months apart in order to help us maintain your oral health. Membership is non-refundable and cannot be combined with dental insurance or other discount plans.
